I consent to my child’s participation in the NYC High School Student Trip.

I, the undersigned parent/guardian, give permission for my child to attend the NYC High School Student Trip. In case of emergency, I authorize the trip leaders/school staff to obtain medical treatment as deemed necessary and accept responsibility for related expenses.

Release of Liability

I release this organization or person(s) from all liability and responsibility.

I acknowledge that participation in the NYC Student Trip involves certain risks, including travel, public spaces, and scheduled activities. I agree not to hold WindowsWear Inc., the school, trip leaders, staff, or chaperones liable for injury, accident, illness, or loss of property. I assume full responsibility for my child’s participation.

Code of Conduct

I acknowledge the code of conduct for this activity or event.

I understand that students are expected to follow all rules and behave respectfully throughout the trip. Prohibited behaviors include possession of drugs/alcohol, leaving the group without permission, or disruptive conduct. Violations may result in disciplinary action and possible early return home at the parent/guardian’s expense.

Assumption of Risk

I acknowledge the potential risk of participation in this activity or event.

I acknowledge that participation in the trip involves inherent risks, including but not limited to travel, transportation, public spaces, walking tours, recreational activities, and interactions with the general public. I understand these risks may include injury, illness, accident, property damage, or other unforeseen events. By signing this form, I knowingly and voluntarily assume all such risks on behalf of my child.

Indemnification

I waive my right to sue this organization or person(s).

I further agree to indemnify and defend WindowsWear Inc., the school, its officers, employees, and volunteers against any claims, damages, liabilities, or expenses (including attorney’s fees) arising from my child’s participation in this trip.
